Mafia III’s important missables come from decisions, not hidden one-time collectibles. Racket bosses can be recruited or killed; informants can be spared or killed; districts can keep all underbosses loyal or drive them into betrayal; and each underboss has favor content that disappears if that character is lost. Plan two campaigns and keep milestone saves rather than trying to preserve every outcome in one run.
The October 2026 patch improved tracking, but it does not remove mutually exclusive choices. A fixed counter still cannot record “kill” and “recruit” for the same person in one story state.
Protect five milestone types
Before a racket boss
Save before entering the final site. Confirm whether this run recruits or kills, and wiretap the correct sector for recruitment.
Before every sit-down
Preserve the district decision so an unintended reassignment or betrayal can be reversed without replaying earlier rackets.
Before the final church choice
Keep a backup before leaving or returning to the plantation, especially if you want several epilogues.
Before long DLC checks
Save before a qualifying Herbalism sale, the final Sammy's upgrade, and any action that consumed hours of setup.
Store backups in a dated folder outside the active save directory. Copy them only while the game is closed, and let Steam Cloud finish before deciding which version to keep.
Split racket bosses and informants across runs
The New Boss rewards recruiting the available racket bosses. Wiretap their sectors and choose the flip response. No Loose Ends requires the opposing lethal behavior. Dedicate Run A to recruitment and Run B to killing.
Informant achievements follow the same logic. Live Another Day and Can’t Trust a Rat reward opposite treatment across the relevant informants. Write “spare” or “kill” at the top of the run checklist and apply it consistently.
Do not rely on reloading the same encounter after one choice to build two cumulative totals. Steam may record an unlock, but the campaign cannot carry both sets of people forward. A second focused run produces a coherent state and avoids uncertain counter behavior.
Keep or remove underbosses deliberately
We’re in This Together and Family belong to the coalition route. Give Cassandra, Vito, and Burke two contested districts each, complete their personal favor chains, and avoid taking assigned rackets away during sit-downs.
Just You and Me belongs to the opposite route, where Lincoln finishes with one underboss. Award the six contested districts to the chosen leader and let the others’ relationships break as the story intends. Keep that behavior off the balanced save.
Trust and the individual favor achievements—IRA Don’t Ask, I Need a Favor, and .45 in My Hand—need the relevant character alive and their optional work available. Speak to every leader after district awards and finish new stages before the last sit-down.
Watch approach-specific district conditions
Achievements such as I’m Goin’ In, Softened ‘Em Up, and Never Saw It Coming relate to how racket targets are exposed or confronted. The journal shows whether informants and enforcers remain and how much earn damage is still required.
Before finishing a racket, read the exact current Steam description and choose the required approach. A walkthrough written for another platform can help locate the target, but use Steam’s words for the condition.
Keep a save before the final damage action. If the achievement requires avoiding or completing a preparatory step, a checkpoint after the boss appears is already too late to change the district’s setup.
Treat earn and cumulative conditions as route-dependent
Big Earner depends on building an underboss’s earn, which conflicts naturally with a perfectly even map at the highest milestones. Decide whether it fits the single-underboss run and complete trafficking missions for that leader’s rackets.
Cumulative achievements for stealth takedowns, brutal takedowns, explosives, vehicle actions, or weapon use are not story missables in the same way, but old tracking problems made them feel missable. On the current build, count fresh actions, save normally, and spread them through both campaigns.
If a count appears not to move, stop repeating it indefinitely. Confirm the target qualifies, update Steam, verify files, and test a clearly valid action after restarting the game.
Reserve DLC checkpoints
Faster, Baby! has story and Herbalism conditions. Preserve a save before the high-value crop sale and before buying the final grow-house improvements. This protects the hours invested in rank and strain development.
Stones Unturned is largely linear, but make a save before its final operation and optional post-story cleanup. Sign of the Times should have one before the concluding cult sequence and another before the final Sammy’s bar purchase.
The DLC achievements are included in Steam’s 85 total. A base-game story completion with unresolved DLC entries is not yet 100%, even if an older guide separates the lists.
Recover from a missed decision
If a racket boss received the wrong outcome, reload the pre-boss save. If an underboss has already betrayed Lincoln, return to a pre-sit-down backup and change the territory plan. The final ending checkpoint cannot resurrect that relationship.
When no suitable backup exists, move the condition to the second campaign. Replaying only the necessary rackets and story chapters is safer than importing someone else’s saves, which may contain different IDs, progression, and achievement state.
For a condition met before October 8, 2026 that remains locked, perform it again on the updated build where possible. Do not assume the patch retroactively awards every historical action when an old save loads.
